Highlights
Part 1
Context
You are required to research, develop, and plan the implementation of a Well-being (inclusion plan) for a child with additional needs who will be entering your service. This section should provide a concise overview of the child’s current competences and serve as the basis of the child’s program planning and service delivery for the upcoming year. Address the areas below.
How the child's developmental delay or medical condition affects his/her access to and participation in age-appropriate experiences: Include any Medical History (if relevant) i.e. Physical, hearing, vision, CDC report, etc.
Detail what they love to do, who do they enjoy spending time with etc? This will come from the parents and child.
Consider how the child's developmental delay or medical condition might affect their access to and participation in your setting. Consider and prioritize the necessary supports that adults need to incorporate for the child to full access and participate freely and equitably in your setting (within a regular early childhood setting with his/her similar-age peers and/or within their home environment.
Any allied/health, additional programs, access to external support systems, knowledge and financial considerations for the families etc.

Case Study
Taya has been attending a children's education and care service since she was 12 months old and is soon going to be turning 4 years old. Taya's family is keen for her to move up into the preschool room from the 2-3 year old room, and they have been talking with the service about transitioning her as soon as possible after her birthday. Taya has a physical disability that requires her to use a wheelchair and she currently wears nappies to meet her toileting needs. The service is unsure if transitioning Taya is the right choice as she is not toilet trained. The transition policy of the service states that all children must be out of nappies and able to use the toilet independently before being able to transition to or enrol in the preschool room.
The service has given Taya's parents a copy of their transition policy and explained that they are unable to move forward with the transition at this time. Taya has been asking her parents if she can move up after her birthday as some of her peers and friends have already transitioned. Taya's parents are frustrated and upset that their daughter may miss out on the opportunity to transition into the older room with her peers.
