Amir, aged 7, comes into the public Primary Health Care Service with symptoms associated with an ongoing middle ear infection in both his right and left ear. Amir’s ear infections have worsened since last week. You inform Amir’s family that he will need to be transferred to the hospital, 30 kilometers away to get further treatment. Amir’s mum, Ashley who is Nepalese and English is her second language is not convinced that he needs this level of health care service and her family’s previous experiences with the hospitals were unfavorable. Ashley gestures that she just wants some more antibiotics for Amir’s ear infection. The General Practitioner has provided a referral and is adamant that Amir needs to go to the hospital for treatment.
