read the patient notes so you can familiarise yourself with the patient’s case. In particular the NUM wants you to identify any issues related to the social determinants of health that may impact the patient both on admission and discharge e.g. where the patient lives, who they live with etc.
Include in your case study response you will need to provide;
1. a brief description of the patient background (this is found in the patient notes
2· a brief explanation of the social determinants of health and why they are significant to patient care and outcomes.An explanation of how issues arising from this case study relate to three social determinants of health

Patient Background: Mrs. Eileen Poole is 74 years old and lives alone, her husband died six months ago. Since that time she says that she has “lost her appetite”. While she is usually fairly active in her garden and manages well at home, she does not drive so she does not often leave home as she relies on public transport to go out. She was born in Scotland and came out to Australia with her parents as a young girl. When she married her husband of 51 years ‘Jim’ she moved to a rural town called Wiimali 4 hours from Sydney. Mrs. Poole has two children, Lauren (35 years) and Jason (32 years). Jason lives in Western Australia and Lauren lives with her husband and their 3 children 5 kilometers away. Mrs. Poole and Lauren are close and speak on the phone every day.
