Question 2 
In this question you are required to conduct a DFE practice on the stapler investigated in Question 1. The tasks include:
a) List at least 10 types of environmental impacts over the life cycle of your stapler covering Materials, Production, Distribution, Use and Recovery;
b) Suggest at least five ways to reduce its environmental impacts with the implementation of DFE design guidelines; and
c) Conduct a Qualitative Life Cycle Assessment (LCA) on the stapler analysed using the template table from 3M.

In this question you are required to perform an FMEA analysis based on the pen investigation in both Questions 1 and 2 and the tasks include:
a) Identify EVERY Way in Which Part Can FAIL; i.e., determine the Failure MODES; and
b) Analyse the CONSEQUENCES of Every Failure; i.e., determine the EFFECTS.
The MS EXCEL Spreadsheet available in the vUWS site only shows you a template to generate an FEMA report. You may create the FEMA Report using a spreadsheet based on this template.
Question 4  Risk Assessment:
In this question you are required to perform a risk assessment for the stapler investigated in Questions 1-3, which may be readily available to students both at home and university as a stationary instrument, the risk level, or the category rating, or safety integrity level, or performance level via using a three-step procedure,
