Case 1
Joyce Coombe is a 63-Year-old lady who presented to the Emergency dept. last night.
Joyce has been suffering with a cold for the past week. She has an irritating cough and is coughing up green sputum. Joyce is an asthmatic and the cold has exacerbated her symptoms. She presented with shortness of breath, wheezing on inspiration, she is flushed and sweaty (diaphoretic) and has a temp. Of 38.5deg. She is complaining of sore chest muscles from coughing, she has a headache and achy joints. She appears and feels exhausted.
Joyce has a history of Type II Diabetes, Asthma for 20 years and Osteoarthritis. Joyce is a smoker and smokes 10 per day. She has been trying to reduce this with not much success.
Surgical history includes a Cholecystectomy 25 years ago.
She suffers with depression and is currently not on any medication for this.
After some initial tests were performed Joyce is diagnosed with infective bronchitis and she is admitted to your respiratory ward for rest and treatment.
Tests performed in the emergency dept.

• Chest Xray
• FBE (full blood examination)
• HB (Haemoglobin)
• Sputum specimen
• Urine specimen
• Treatment orders for ward management.
• The medical orders are for nasal specs oxygen @ 2L/min to maintain oxygen saturations 90-95%
• Spirometry readings to be done before and after nebulisers
• A high protein, diabetic diet has been ordered
• Joyce is to be encouraged to increase her intake of oral fluids.
• Twice daily BGL’s
• She is to be commenced on a fluid balance chart and diabetic food chart and is to have second daily weighs.
• Access the RDR chart, FBC chart and medication chart and consider the data provided.

• Raphael is a 28 year old Sudanese woman who presents to your community clinic with abdominal pain, she has been experiencing this pain for the past week. She looks unwell and is extremely anxious and frightened. Raphael is fearful that she could die. She doesn't speak English. Apart from her husband and their two children, aged six and two years, she does not have any friends or relatives in Australia. Raphael is also worried about who would look after her family if anything happened to her. Her husband and the two children have accompanied her to the clinic and they do not want to leave without her.

• You are a first year student nurse and you answer the call bell of an elderly male patient you do not know. When you enter his room he is sitting in his chair, when you ask him what he needs he points in the direction of the bed indicating that he wants to get back into bed. You know from handover that he has been diagnosed with a right sided CVA (Stroke). You observed him being moved the previous day by another nurse, so thought this would be an easy task to attempt yourself. You position yourself in front of the patient and ask him to lean on you and stand up. As you do this you realise the patient has no strength in his legs and he has weakness down one side of his body. You think he might fall, so you sit him back down again.
