Incident Report
For this assignment you will create an incident report out of the narrative account provided. The goal will be for you to create a professional, objective report that follows the format found on page 30 of WFCHP. It will be graded for its ability to follow the COCOA CAT principles (WFCHP 18-28).

Incident Narrative
It was a rainy day on Tuesday September 27th, 2016. I was working in the kitchen of the college, and was preparing the ingredients for the chili special that we would be offering the following day. My co-worker, Willy Makit, was also in the kitchen with me. Willy frequently needs to use the washroom and sometimes has to make a run for it. I was chopping onions when I heard Willy yelp in pain. He came over to me and asked for a Band-Aid because he had cut himself. Willy is a typical twenty seven year old male, and he sometimes makes a bigger deal of things than he needs to – like a soccer player. It was then that I noticed his hand.
He had his thumb wrapped in a yellow cloth, and the cloth was soaked with blood. It was like something out of a horror film. I said that we needed to call a manager. The manager was squeamish, so he took Willy to a chef to take a look at the wound, and the chef said Willy should go to the hospital. The manager drove Willy to the hospital and asked him how it had happened.
Willy had explained he was being stupid while cleaning knives. He was cleaning the knife with the blade toward his hand. The serrated bread knife slipped and cut his thumb.
When Willy arrived at the hospital it was still raining outside, and Willy felt cold. After checking in with the nurse he was hungry so he bought a bag of chips from the vending machine, and our manager went outside for a smoke. The hospital had a polished green tile floor, and it smelt of cleaning products. Later, Willy had told me he wished his house was as clean as the hospital. At the hospital Willy was told that his cut would need stiches. The doctor wanted to give him anesthetic to freeze the finger, but Willy is afraid of needles so he refused. They had to apply a topical anesthetic instead of an injection. Willy had to wait a half hour for the anesthetic to take effect.
