Scenario
Ms. A works as an Enrolled Nurse in a residential care setting/facility. Ms. A is working on the morning shift and has been delegated by the registered nurse to care for Nellie Roma, who is in the early stages of dementia and has some memory loss. Nellie has been assessed as a high fall risk. Ms. A enters Nellie’s room, introducing herself, greeting Nellie. Ms. A gains verbal consent from Nellie to assist her with her morning shower. Ms. A asks Nellie to select the clothing she would like to wear. Ms. A assists Nellie to the shower and closes the bathroom door and helps Nellie to complete all her hygiene care, communication with Nellie throughout the cares. Ms A mobile phone rings, but she does not leave the shower or answer the phone and apologizes to Nellie for the interruption. Nellie said she had mislaid her glasses, so Ms. A says she will find them later. On return to the bedroom, Ms. A ensures Nellie is safely seated and then looks for Nellie’s glasses. Ms. A documented the care given and returned the notes into the nursing station.
