Instructions:

For this assignment, you will create a basic balanced scorecard for your organization. 

For this assignment, you will need to understand the following domains of quality and how quality can be measured to assess those domains. Choose two domains you want to address for your fictitious organization:

• Access
• Efficiency
• Effectiveness
• Safety

Requirements

Your Excel sheet must include:

a) The name of your 2 specific indicators, organized under the domain to which each pertains.

b) A definition of each of the 2 specific indicators that explains the indicator and how it is measured – including a formula if relevant for your indicator(s).

c) The data source(s) for each of your chosen 2 indicators (e.g., the Discharge Abstract Database (DAD)), 

d) The target for each of the 2 specific indicators.

e) The frequency with which the 2 specific indicators will be reported. 

f) A visual method to display results the data for the 2 specific indicators, over time, in the frequency you chose. Your visual display method must have a way of showing readers whether the results are at, below, or above the target (such as the “stoplight” method).

Note: It is not necessary to provide fictitious data – you are building the scorecard framework. Therefore, you don’t need to put results into the scorecard, but it will need to have a way of showing the reader how results will be displayed, such as a legend or key.

Requirements

Your report must include the following:

a) A definition, in your own words, of all four domains of quality identified at the start of the assignment. 

b) Which two domains of quality do you choose to address and why? 

c) The process you undertook to research and choose the 2 specific indicators. 

d) An explanation of the data source(s) that would be used to measure each indicator (for example, if your data is coming from the DAD, explain which fields or elements you need to utilize to calculate your indicator). 

e) Your rationale for choosing the 2 specific indicators including why you feel these will be an effective measure of that quality domain (i.e., why will this help your organization assess quality in those domains?). 

f) Your rationale for choosing the targets for each of the 2 specific indicators, including why you feel that is a reasonable and appropriate target. If your target is based on an external benchmark, explain the source of the benchmark.

g) Your rationale for selecting the frequency of reporting the 2 specific indicators.
