•  She has an abdominal wound and a wound on her right leg.   
•  A CAT scan rules out a head injury.   
•  Her mother sits with her all the time due to her autism.   
•  Her father is admitted in the same hospital in stable condition and is being monitored for a head injury. 
•  She has orders for no concentrated sweets due to Type II diabetes.   
•  NKDA, 
•  immunizations are up to date.   
•  She is able to take her medicines orally.    
•  BP 138/80, 20 RR, 84 p, 98.7 temp. 
•  Lab results from admission:  H&H 14.0 & 36 
•  
• Doctor’s orders 
• Metformin 500 mg BID, PO 
• Ativan 5mg po every 8 hours as needed for anxiety. 
• Dilantin 100 mg TID PO, clinically effective serum level 10-20 mcg/mL 
• Start IV: Lactated Ringers 125 cc’s/hr.  Give 500 cc’s bolus of fluid prior to transfer to OR. 
• Labs:  CBC, Type and Screen, CMP, Dilantin level, Hemoglobin A1C 
• Nitroglycerin prn sublingual for chest pain 
• Percocet 5/325 1-2 tabs po every 4-6 hours as needed for pain.  Give pain medication one hour prior to
• dressing changes. 
• She is scheduled for surgery on her abdominal wound this afternoon at 2:00 pm.   
• Place urinary catheter to straight drainage by 1:00 pm for surgery. 
• Give bed bath if patient does not want to use the shower 
• No concentrated sweets 
• Seizure precautions
