Sarah was a young woman who had a forceps delivery at 41 weeks+4 days for failure to progress, which accompanied liquor with thin meconium and an estimated blood loss of 500ml. Her male infant was born healthy with no complications and remained with her following the birth. Antenatally, small symphysio-fundal height measurements, and two episodes of reduced foetal movements had caused her significant anxiety. Her history showed no previous or family history of mental illness, although she had a brother with significant learning disabilities. She was of a non-English speaking background; however, she spoke English well and was employed working full-time night shift as a nurse prior to taking maternity leave. Her parents and family, with whom she communicated often, remained in her home country after her migration several years ago. Her husband spoke much less English and required interpreting assistance. A challenging housing situation was also discovered during her admission. The couple rented a room in a five- bedroom house along with four other couples, two of whom were recently postnatal.. After giving birth, Sarah was discharged to community midwife care on day 1. On day 10, the midwife noticed some bizarre behaviour exhibited during the appointment. Specifically, Sarah was restless and agitated, her thoughts were disjointed, and she did not fully believe that her baby (Aden) was hers – she believed something had gone wrong at the hospital and she had been given another woman’s child. Despite this mix-up, she said she was continuin  to breastfeed the baby. She appeared very suspicious and paranoid, telling the midwife that people were judging her and watching her through the windows. Sarah told the midwife that she was not bonding with Aden, as he was not her baby anyway. She then whispered to the midwife that she had thoughts of harming him. The midwife organised for Sarah to attend the local Emergency Department and Sarah was then admitted to the acute inpatient mental health unit.
