Case Study
Linda Halliday is a 78-year-old patient who lives on her own but has a family who live 1 hour away, and phone on a daily basis. She has a history of hypertension, atherosclerosis and angina.
Linda has presented overnight to the emergency department with orthopnoea, ‘crackles’ on auscultation in both lung fields; and peripheral oedema of both legs to mid-calf level +2, shortness of breath on minimal exertion and during conversation.
In the emergency department a peripheral intravenous cannula was inserted into the cephalic vein on the anterior left forearm. She was given a stat dose of IV Lasix 80 mg via a peripheral Intravenous cannula, resulting in a diuresis of 500mls.
Linda has been admitted to the medical ward with acute congestive cardiac failure.
You take over her care on the morning shift, and after introducing yourself you prepare to administer her medications. The order is as below 40 mg Lasix PO. Potassium Chloride 600 mg PO.

You prepare the medication following the ‘rights’ of medication administration, but as you go to hand the medication to Linda, she tells you she will not take this medication.
