 On admission to the ward his observations included; T 36, R 30, P 104, BP 90/60 and SAO2 90% on  RA. You note he is dyspnoeic and pale.
 He is concerned about where he will be able to smoke in the health care facility and is anxious to be  discharged as soon as possible as he is playing with his basketball team tomorrow.  He is accompanied by his heavily pregnant wife who is anxious and their two year old son.  A carcinoma of the bowel is suspected.
