Introduction
Before starting work: lease read this entire document. It contains helpful information about the assigned assessment tasks and how to present your work.
This assessment covers the following Units of Competency (UOC/s):
HLTAAP002 Confirm physical health status
This unit of competency (UOC) forms part of the HLT54115 Diploma of Nursing offered by NSW Health RTO, and students must complete all assessment items to be deemed competent.
Unit Descriptors
This unit describes the skills and knowledge required to obtain and interpret information about client health status and to check a client’s physical health. It requires a detailed knowledge of anatomy and physiology.
This unit applies to individuals working directly with clients and who assist in the provision of health care services. Some disciplines may be subject to state/territory regulatory determination regarding delegation and supervision.
The skills in this unit must be applied in accordance with Commonwealth and State/Territory legislation, Australian/New Zealand standards and industry codes of practice.
Assessment Requirements
For more information relating to the RTO assessment requirements, including the management of student progression, please refer to the NSW Health Student Handbook. For course specific assessment requirements, please refer to the Course Guide.
Students are required to complete all the assessment tasks included in this document. The assessment tasks are summarised in the Assessment Plan in this document.
Your assessor will discuss all of the assessment requirements with you prior to undertaking this cluster. The deadline for completion of this assessment will also be discussed at this time.
You can hand in your completed assessments before it is due. If however, you cannot hand it in by the due date, you will need to negotiate with your Program Coordinator prior to the assessment due date (except in circumstances that prevent you from doing so).
Should you fail to submit your assessment on time and without prior approval for an extension, after a period of two weeks you will be deemed not competent and your manager will be notified of this outcome. If at a later date you are able to negotiate the resubmission of your assessment with your Program Coordinator and you are deemed competent, this outcome will be changed.
Following submission of your assessment, you will be provided with feedback and if required, further training may be arranged to facilitate achievement of competency.
If on second attempt you are unable to meet the requirements, an interview will be held with your Program Coordinator to assist further development and achievement of competency.
The Student has the right to appeal an assessment decision at any time. (Please refer to NSW RTO Student Handbook for the Appeals process).
Your assessor will advise you of the due dates for each of the assessments, or in the case of a workplace observation/s, the date/s and time/s of the observation.
Assessment Plan
Guidelines and instructions are provided for each assessment activity throughout the document. Please familiarise yourself with these requirements prior to undertaking them.
Assessment Task 1 – Short Answer Activities/Diagrams
This assessment task will assess your knowledge of anatomy and physiology of the eleven (11) body systems in the human body. You will need to demonstrate understanding of the eleven (11) body systems, correctly label diagrams and demonstrate a basic understanding of medical terminology.
Assessment Task 2 – Weekly Mini Quiz
This assessment task will assess your knowledge of anatomy and physiology of the eleven (11) body systems in the human body. This assessment will be based on the content learnt over the semester in four (4) quizzes scheduled as per your campus timetable. You will be required to gain 70% for each Mini- Quiz in order to pass. If you do not obtain 70% on your first attempt, you are to meet with your assessor for feedback. You will be given a second attempt to obtain 70%. The Course Coordinator will arrange a date and time. All students will be given feedback on their examination regardless of the mark to identify deficit areas.
Assessment Task 3 – Case Study: Urinary System
This assessment task will assess your knowledge of anatomy and physiology of the Urinary System. You will be given a case study of a patient with Chronic Kidney Disease. You are to answer six (6) short answer activities.
Assessment Task 4 – Concept Map
You are to choose one (1) disease/disorder for one (1) patient you take care of during your work placement. You are required to design and develop a concept map to demonstrate your understanding and knowledge of common disease states, anatomy and physiology of these disease states, and the care needed for individuals with these conditions.
Assessment Task 5 – Examination
This assessment task will assess your knowledge of anatomy and physiology of the eleven (11) body systems in the human body. This assessment will be based on the content learnt over the semester. You will be required to gain 70% in order to pass. If you do not obtain 70% on your first attempt, you are to meet with your assessor for feedback. You will be given a second attempt to obtain 70%. The Course Coordinator will arrange a date and time. All students will be given feedback on their examination regardless of the mark to identify deficit areas.
Assessment Task 6 – Simulated Skills Assessment
During this Semester you are given access to the simulation laboratories to participate in clinical experience of undertaking a physical assessment including taking vital signs such as Blood Pressure, Pulse, Respiratory Rate, 02 Saturations, Temperature and Blood Glucose monitoring. You are required to be assessed as per competency requirements in the Simulation Laboratory Book 1, prior to clinical placement.
Assessment Task 7 – Work Placement
For this assessment task you are required to successfully complete two (2) weeks of clinical placement throughout Professional Experience Placement (PEP) 1.
Assessment Task 8: Third Party Validation
For this assessment task, you are to have your nursing unit manager (NUM) or clinical nurse educator (CNE) complete your performance evaluation in PEP 1.
* When assessing the learner/student by observation, the learner/student must be observed MORE THAN ONCE.

Define Passive Transport. Provide two (2) examples of passive transport
Passive transport involves movement of water molecules from higher concentration to lower concentration. This transport is ATP independent; do not require energy for the transport. Eg. Exchange of gases in the lungs, exchange of nutrients in the kidney.
Activity 12
Define Active transport. Provide two (2) examples of Active transport.
Active transport involves movement of water molecules from lower to higher concentration. It is ATP dependent transport. Eg. Sodium potassium pump, uptake of mineral ions by roots of plant.
Activity 13
Outline the four (4) tissue types and provide an example for where each would be found.
Four basic types of tissues are:
1. Connective tissue – bone blood, lymph tissue.
2. Epithelial tissue – skin, gastrointestinal lining.
3. Muscle tissue – stomach muscle helps to move skeleton.
4. Nerve tissue – brain spinal cord. Carry messages to and fro to the whole part of the body.
The Integumentary System

Explain what constitutes the integumentary system.
The integumentary system consist of:
1. Skin
2. Skin appendeges – hair, nail, sweat gland, and sebaceous gland
3. Subcutaneous tissue
4. Mucocutaneous junction
5. Breast
Activity 15
Identify three (3) functions of body membranes.
1. They eliminate toxic substances from cells.
2. Body membrane consists of receptors and channels which allows selective entry of substances and molecules.
3. They separate vital metabolic processes carried inside the cells from outer environment.

Gastric Oesophageal Reflux Disease (GORD)
In this acid produced in stomach leaks up in to oesophagus.
Oesophagus Heartburn, acid reflux, oesophagitis, bad breath, bloating and bleaching, pain while swallowing. It includes self help measures such as small meals but should be frequently taken and medications as prescribed.
Ulcerative Colitis
In this disease colon and rectum becomes inflamed and can develop ulcers.
Colon of large intestine and rectum. Recurring diarrhoea wthich may contain mucus, blood, or pus, abdominal pain, need to empty bowels regularly, painful and swollen joints, mouth ulcers, irritated and red eyes. Medications may include anti-inflammatory drugs - corticosteroids, immune system suppressors - Azathioprine, biologics – infliximab.
Cholelithiasis (Gallstones)
It is hard deposition of digestive fluids in gallbladder. It may occur like stones which varies in size.
Gall bladder Nausea or vomiting, backpain between your shoulder blades, pain in right shoulder, intensified pain in upper right portion of abdomen, gallstone may pain from few minutes to several hours. Cholecystectomy- surgical removal of gall bladder, medications to dissolve gallstones.
Hernia
It is abnormal abdominal bulging of any organ or tissue from its cavity where it resides. Hernia has number of types.
Belly bottom, upper stomach, bladder, intestine and others can be affected. Visual lump in the affected area can be seen. Early medical care, change in lifestyle, attempting surgery if needed, but if left untreated can threat life.
Pancreatitis
Its inflammation in pancrease due to gallstones.
Pancreas Upper abdominal pain that even radiated to back, feels worse after eating, rapid pulse, nausea, fever, loosing weight, foully oily stools. Fasting, intravenous fluids, procedures to remove bile dcut obstructions, gallbladder surgery, pancreas surgery may help to treat.
The Endocrine System

Outline the functions of the endocrine system.
An endocrine gland plays a vital function to secrete hormones in the blood stream further it targets to destined organs or any part of body. Endocrine glands are chemical compound whose receptors are present on target cells they can either cell surface receptor or within the nucleus. Hormones serve as messengers who serve in control and coordination. They control various ultimate body functions such as growth and development, control entire organ, development of sexual characters, feedback mechanism, etc. Some effect one or two organs and many has influence to entire body parts. Other major functions also include controlled cell growth, secretion of insulin and other chemicals such as rennin in the blood, control heart rate, etc.
Example gonadotrophin hormone (GTH), thyroid stimulating hormone (TSH), growth hormone (GH) is secreted by anterior lobe of pituitary gland which targets development of gonads, thyroid, cell growth of body respectively.

Hormone levels are controlled by negative feedback. Whenever a hormone is produced and secreted, the release of hormone is elicited by stimulus when requirement is fulfilled or its level is reached to serge a negative feedback is generated such to stop the secretion of particular hormone. In this way the concentration of hormone is maintained in blood.

List the various cells in the Islets of Langerhans. Describe a function for each.
Islets of Langherhans comprises of clusters of cells which comprises of mainly three kind of cells. 1. Alpha cells (?) - secrete glucagon, releases fatty acid from fat tissues and glucose from liver.
2. Beta cells (?) – secrete insulin, regulates carbohydrate, fat and protein metabolism, inhibits the secretion of glucose from liver.
3. Delta cells (?) – secrete somatostatin, act as strong inhibitor of somatotrophin, insulin and glucagon and its function is not crystal clear yet.

- Maturation of T lymphocytes, brings about cell mediated immunity and graft rejection.
- Give rise to stem cells from which various types of blood cells are formed, development and maturation of B lymphocytes, source of antibody synthesis.
- Initiates and develops humoral and cell mediated immune response, important centre of phagocytosis.
- Brings humoral and cell mediated immunity, act as reserve tank for RBS formation at the time of necessity, acts as filter for foreign particles, graveyard for worn out blood cells.

Identify and compare the differences between T cells and B cells.
The differences between B cells and T cells are as follows:
1. B cells mature in the bone marrow where as T cells mature in the thymus.
2. B cells bring about humoral immunity where T cells bring about cell mediated immunity.
3. B cells produce antibodies but T cells produce lymphokines.
4. B cells can bring about immediate type of hypersensitivity where as T cells can bring about delayed type hypersensitivity.
5. On activation, B cells produce plasma cells and memory cells. T cells produce sensitized T cells and memory cells.
6. Lymphocyte capping, which occurs during the differentiation of B cells is not found in the differentiation of T cells.
7. B and T cells differ in their activation by different mitogens.

You are not allowed to have another person login on and complete the weekly mini-quizzes on your behalf. This will result in disciplinary action and you will be referred to the student review panel.

You will be given your results straight away.

Strict examination conditions will be implemented during this assessment (see Student Information Handbook).

Mr Brian Smith has been admitted to the renal ward for investigation and management of stage 4 Chronic Kidney Disease (CKD). Brian has a medical history of polycystic kidney disease, diabetes and hypertension.

You will participate in a simulated clinical experience in the following specialised skills:

You will be required to practice these skills three (3) times in the simulation laboratory prior to assessment.

You are to arrange for your NUM or CNE to complete your Performance Evaluation – Summative Report, located in PEB 1, during each block in PEP 1.
