Case study 1
Mrs Dora Smith is an 78 year old lady who has just been admitted with increasing shortness of breath over the last 2 weeks. Mrs Smith stated to the admitting nurse that she noticed she has been more tired than usual and gets breathless just do normal things around the house. 

Dora has a past history of MImyocardial infarction(6 months ago), osteoporosis, and a fractured L) NOFneck of femur 2 years ago. Dora has smoked a packet of cigarettes a day for the last 50 years and continues to do so.

So far your shift has been as follows:
• Dora requested an early shower. She was quite short of breath and need assistance to dry her back and feet. 
• Dora refused breakfast stating she wasn’t hungry. She drank cups of tea throughout the morning. 

During your nursing assessment you find the following: 
• SpO2 93% on room air
• RR 25/minute
• Frequent moist cough, yellow sputum, audible wheeze
• T 37.8high temperature
• Auscultation chest = crackles 
• Blood test – elevated white blood cell count( trying to fight off an infection)

Q1. Identify the current pathophysiology in relation to the above patient data - what is wrong with this patient?  Explain why you came to this conclusion.

Q2.  What underlying condition do you think she has which would have increased her risk of developing this condition?Why do you think she developed it?

Q3. Develop two nursing diagnoses for Dora.  The nursing diagnoses must have an existing or potential problem that she has or may experience related to a medical condition.  For each diagnosis develop two nursing interventions and provide a rationale (reason why the intervention will be effective) for each.
