Scenario
Mrs Patricia Bond (or Pat, as she likes to be called) is an 84-year-old woman who has been living on her own since her husband passed away two years ago.
Pat has become less able to care for herself over the last six months. An Aged Care Assessment Team (ACAT) assessment determines that Pat would be a candidate for an aged care facility. She is moved to a facility close to her son so her family can visit regularly.
On admission to the age care facility, a holistic health assessment is performed on Pat. The details of this assessment are provided in the table below.
