Scenario – Lowanna AjabathaMrs Lowanna Ajabatha a 50-year-old woman who presented at the emergency department with right (R) upper quadrant abdominal pain with associated nausea and vomiting. She has been admitted for further review of possible cholecystitis and pain management.
Past medical history: Hypertension, asthma, appendectomy, hysterectomy and GORD.
Medications: Seretide 250/50mcg inhaled BD, Coversyl 10mgs oral daily, pantoprazole 40mg oral nocte, vitamin D3 1000units oral daily and paracetamol 1g oral as required.

Social history:

Aboriginal and Torres Strait Islander woman who lives alone in a two-storey apartment. She is divorced and has no children, but has a cat named Fluffy.
O/E:?RUQ pain 8/10, nausea & vomiting, mainly bile fluid.?BP 155 / 90,?Pulse 100 regular,?Respirations 18,?Oxygen Saturation 99% on RA and Temperature 38.5 degrees. She weighs 110kgs.

Mrs Ajabatha is sent for a CT scan of her abdomen and the diagnosis of cholecystitis is confirmed caused by cholelithiasis. She has been commenced on IV metronidazole and ampicillin to control the infection however she continues to have severe right (R) upper quadrant abdominal pain and a high temperature. After a surgical review, it has been decided she requires a cholecystectomy and has been scheduled for the following morning at 0800hrs.
