Mr John B is a 68-year-old retired Irish Anglican Priest with mild Alzheimer's Dementia.  

He lives with his daughter, Leah and her 2-year-old son since his wife passed away one year ago. Recently Mr B has had issues with stability, which resulted in a fall. 

Mr B was admitted to the Rehabilitation ward following a repair of his L) #NOF (Neck of Femur).  He was extremely anxious and refused to engage in any rehabilitation exercises. He also refused to speak to any health professionals regarding follow up care and community home care, following discharge from hospital.  

Mr B's daughter is a single mother and works only part-time as she stays home to care for her father because he refuses to go into a nursing home. He also refuses any home help nor does he go to church anymore. He also refuses to go to the senior community centre for day time activities, after he was told he will not be welcome for his behaviour, as he cursed at the people.

History:  
Mr B suffers from early Alzheimer's Dementia, hypertension, Hyperlipidemia and Depression.

Social:  
Leah states that due to his stubbornness and change in behaviour they no longer go out anywhere to socialise as a family. She stays home to care for him on her days off as she fears for his safety if left alone. Leah is concerned that her father's mental condition has deteriorated but he feels he is fine. 

She also states that he mentioned on multiple occasions that he feels he is a burden on Leah as she takes time off work for him.  

He also has not been compliant with medication and refuses to attend his GP appointments. Leah also states her concern for his ongoing health and also the stability of her job if she takes many days of work.

Using the case study above, answer the following questions: 
1. Discuss the conclusions drawn from the health data.  
2. Discuss the physical, psychological and social impacts of the health issues on Mr B and his family.  
3. Identify health literacy issues that potentially inhibit Mr B's capacity to access, understand, evaluate and communicate information in order to maintain and improve his health. 
4. Identify the priorities and potential risks for Mr B.  
5. What strategies/ interventions can you implement to support Mr B in understanding information and the issues that you identified in Q 4? Ensure your nursing interventions reflect the individual's needs, culture, religion, age, gender, health issues and social needs.   
6. Discuss all possible follow up care Mr B will need following his discharge from hospital. Include possible interdisciplinary teams that may contribute to Mr B's care

Following discharge from hospital, Mr B is not coping and needs wound management care as well as assistance with ADL's.  His daughter, Leah fears for his safety and is also having difficulty coping as she is unsure of what she is doing. You are his community nurse and have been referred to see Mr. B at his home.  
 
7. What nursing interventions can you implement in the community to adjust to Mr. B's needs in his home to facilitate his independence with ADL's? Include family to help assist with Mr B's self-care.  
 
You notice during conversation Mr. B is SOB (short of breath), holds his chest and complains of mild chest pain and of having palpitations.
