Scenario
Consider you are a stage 2 nursing student undergoing placement at an outer metropolitan hospital. You are working with a Registered Nurse (RN) providing care for Mr James Chirica. You read the progress note (below) and note that Mr Chirica lives j in a disadvantaged area in Adelaide, has a low income and has type 2 diabetes mellitus (T2DM), a chronic health condition. The RN suggests that Mr. Chirica's low health literacy may have led to medication errors, resulting ina fall and subsequent hospitalisation.
You discuss this case with your Clinical Facilitator (CF) and they agree that these social determinants of health (SDH) have significantly impacted his health. The CF suggests that you research this further and prepare a report for the next meeting about the impact of these SDH on chronic health conditions, and how diabetes may affect individuals such as Mr Chirica/his family as well as the Australian community. Finally, they ask you to identify strategies that you can use to support the health literacy People with chronic health conditions such as Mr Chirica.
1. Introduction
In this section, clearly state the purpose and structure of the report. Briefly explain type 2 diabetes mellitus (T2DM). Support your explanation of T2DM with at least one (1) scholarly reference.
2. Impact of social determinants of health on chronic health conditions
Chose two of the social determinants of health (SDH) identified in the scenario section andexplain the impact that these factors may have on the health of Mr Chirica and other individuals in Australia. These could include the impact of these SDH on risk of T2DM and other chronic conditions, engagement with health services, adherence to prescribed medications and health outcomes such as complications.
Support your discussion in this Section with at least three (3) scholarly references. This section must use Australian resources.
3. Australian healthcare needs and priorities and the impact of these on the individual and community
T2DM is a healthcare priority in Australia due to the impact of the health condition on both the individual and the Australian community. In two (2) paragraphs discuss the impact of T2DM on:
a) Individuals
b) The Australian community
The impact of T2DM on the individual may include concepts such as mental health problems or social effects. Impact on the Australian community must use Australian resources and may include concepts such as burden of disease, hospitalisations and healthcare expenditure. Support your discussion in this section with at least four (4) scholarly references.
4. The role of the Registered Nurse in enabling health consumers to improve health literacy
From your university studies you understand that health literacy is a SDH that has a significant impact of the health of individuals. In this section, explain two (2) health education strategies a RN can use to enable health co&sumers with chronic health conditions such as Mr Chirica to support their health literacy. Support your discussion in this section with at least four (4) scholarly.
