Respiratory Case Study

Master Noah is a 5 year old boy admitted to the Royal Children’s Hospital Emergency Department in respiratory distress. His parents are in attendance. He was diagnosed with asthma 3 months earlier. It was then that he was prescribed Montelukast 5mg Nocte and PRN Salbutamol as per his asthma action plan. He weighs 21 kg and is 111 cm tall. He has NKDA. His mother has noticed him coughing more than usual and has had a runny nose for the past week.

Admission

On admission to the ED, Noah has a normal mental state, some increased work of breathing with accessory muscle usage and some limitation of ability to talk.

The admitting doctor performs a bilateral respiratory assessment and on auscultation notes inspiratory wheeze with his CXR showing hyper inflated lungs. He is diagnosed with moderate exacerbation of asthma secondary to URTI (Viral rhinitis)

Admission Observations

Temperature -38.3 via tympanic

Pulse rate -128 beats/min

Respiratory rate -32 breaths/min

Blood pressure - not taken

SpO₂ - 90% RA

Medical Orders:

• Salbutamol by MDI and Spacer as per RCH protocol 1 dose (12 puffs) every 20 minutes for 1 hour then review
• Ipratropium by MDI and Spacer as per RCH protocol 1 dose (8 puffs) every 20 minutes for 1 hour then review
• Oral Prednisolone stat 2mg/kg
• Continuous SpO2 monitoring if able
• Sputum MC&S if able
