Case study At age 40 Carolyn was diagnosed with hypertension, elevated cholesterol and prediabetes, and now 5 years later her annual fasting blood glucose indicates progression to type 2 diabetes.

Since adolescence she has been overweight, and tried various diets and weight loss strategies, but has repeatedly regained the weight she lost. Carolyn appears sad and withdrawn, and says she is overwhelmed by the changes required to manage her health.

For this assessment task you are required to address both Part 1 and 2.

Part 1 Discuss the potential social and psychological impact of a diagnosis of diabetes for Carolyn and explain how these issues may act as barriers to optimal self-management of her health conditions. 

Part 2 Drawing on the case study and current evidence-based literature, select one relevant screening questionnaire to assess her psychological status. The selected screening tool should be appropriate for the diabetes educator to use in a consultation with Carolyn.
