Case study 1: Ice Use  
In response to concern about ice use in a rural community you have been asked to conduct an evaluation to better understand the ways in which a community health service currently meets the treatment needs of ice users and what it can do to prevent ice use.  There is an evaluation reference group. It consists of: 
•    Jane is the CEO of the health service she feels that some action needs to be taken to allay community concerns but is uncertain of the best way forward 
•    Len is a respected community member whose son has issues with ice. Len believes the drug problems in the community are due to a failure of the health system in general and a failure of the health service in particular  
•    Marie is maternal and child health nurse at the service she sympathises with Len’s position however she worries that providing more services for ice users will make the service less safe for families  
•    David is a GP at the service he believes that the ice users who attend the service receive appropriate treatment. He believes that the increase in ice use is the result of social factors that are beyond the scope of the health service. He does not believe the health service should change its approach in any way.  
•    Paul is the director of the local drug treatment service/needle exchange he believes their service is underused because of the stigma associated with attending and poor links with the community health service.  

Case study 2: Ehealth record
Australia is planning on introducing an ehealth record. There is fairly clear evidence from overseas that the only viable approach to having a useable ehealth record with sufficient uptake to justify health system change is one where people opt-out of the record. Nonetheless the Australian government has decided to run pilot projects in 4 different sites to compare opt-in and opt-out health records. Key stakeholders include: 
•    The Department of Health who recognises the need for the ehealth record but is concerned about managing risks 
•    Health services who see the value of the ehealth record but are concerned about the level of organisational and technical change required to implement it effectively 
•    Clinicians who see the value of the ehealth record but are concerned that if patients withhold information from the record it may compromise their management  
•    Patient groups who see the ehealth record as an important tool in integrated care but support the right of patients to choose what information is included in the health record  
•    Community groups who are worried about privacy
 
Case study 3: HPV Vaccination
Australia currently vaccinates teenage girls and boys against HPV. The benefits of the vaccine for girls are clear. However the policy for boys is controversial because the vaccine only benefits boys who as men have sex with other men.
You have been asked to evaluate the current policy, provide recommendations for the future and a supporting communications strategy. While the current policy has strong support there is also resistance from a range of sources. The resistance can be characterised as:
•    Evidence based ie there is no benefit for heterosexual men
•    HPV specific ie most vaccines are for diseases that are highly infectious and not treatable. HPV is preventable and its consequences are treatable so this vaccine should be treated differently
•    Libertarian ie vaccine policy is a case of the government imposing its will on the bodies of its’ citizens
•    Anti-industry ie big pharma and the medical industry make money by duping people into thinking they need vaccines that are ineffective and dangerous
•    Religious ie HPV vaccination reduces disincentives to promiscuity thereby promoting risky sexual behaviour
