Introduction of the patient and the knowledge deficit: Patient has Dka transferred from Icu. Vitals taken, BTF except bsl 8.4 mmol . History patient of having type one diabetes since age 13, PMH- reduced mental capacity due to hypoxic brain injury, GCS= 14. Patient also has Oral Amoxit for chest infection. Commencing normal saline and diabetes diet   She has poor understanding about the illness cause she is eating chocolate  on arrival. Needs encouragement for oral fluids intake and mobilization and awareness about the illness and the diabetes diet .

Assess :

1. Patient requires education for her diabetic condition
2. Ensure patient is comfortable and is not in any pain (Well alert for the education)
3.  Ensure the environment is non distractive for the session
4.  Ensure the language is polite and non-controlling and judgement 

 5. Needs encouragement for Oral fluids intake and mobilization and diet
Develop:  

Introduction of Diabetics. Knowing what information, the patient has about diabetes .The patient needs to be made aware about DKA and effects of having chocolates in her health. The symptoms and risks of the diabetes should be explained. Also the ways of reducing the risks due to the disease should be acknowledged.  Hypoglycemia and hyperglycemia should be identified to the patient.  In addition, the patient should be made aware about the food and its effect on BSl. Subsequently, the patient should be encouraged and promoted to have a session with the dietitian for her food intake.  
Plans : Teaching plan : The background information  and potential risks about the disease is explained  to the patient . Awareness about exercise and diet restrictions should be explained. The importance to monitor the BSL and food intakes prior to the medication and after medication should be acknowledged. The patient needs to be encouraged for the dietitian session to have a food plan  so that the risks of hyper and hypo glycemia is reduced. 
Action Plan : Demonstrate the patient the way to check the BSL , monitor weight and ketones regularly .
