1.2. Introduction 
Evidence-based practice is a problem-solving approach to the delivery of care that integrated the best evidence from a body of studies with a clinician’s expertise, which includes internal evidence and patient assessments, and a patient’s preferences and values. This represents a paradigm shift where the emphasis of healthcare is focused on the best outcomes for patients/clients based on the best available evidence. This involves a shift from practice based upon tradition to clinical decision making is supported by evidence from research studies. The goal of this lifelong learning approach is the achievement of outcomes that are right for a particular patient. The decision-making happens within a context of caring, enabling each provider-patient encounter to be as unique as it is and deliver unique outcomes (Melnyk & Fineout-Overholt, 2011). 

Evidence-based practice involves the following seven key steps in Melnyk & Fineout-Overholt (2011) Implementing evidence-based practice to enable developing a precise, searchable and answerable clinical question.
 
• Cultivate a spirit of inquiry 
• Ask the clinical question in PICO (Patient population; intervention; comparison groups, outcome) format 
• Search for and collect the most relevant and best evidence to answer the clinical question 
• Critically appraising the evidence for its reliability, validity and applicability. 
• Integrating all evidence with clinical expertise, patient preferences, and values in making a practice decision or change. 
• Evaluating the effect of the implementation on health care outcomes. 
• Disseminate the outcomes of the EBP decision or change 

Aim of assessment:
The aims of the assessment are for students to: 

1. Discuss the principles and processes of evidence-based practice;
2. Critically discuss the importance of individual patient preferences when making clinical judgements about the implementation of research findings;
3. Critically discuss barriers and facilitators to implementing evidence-based practice in the clinical area;
4. Conduct a comprehensive literature search on a clinical question. 

The purpose of this two-part assessment is to enable postgraduate students to utilise their clinical experience within their learning and skill development associated with critically reviewing the literature as part of evidence-based practice.

PART A: Essay

With reference to your own clinical practice in no more than 1800 words identify the challenges and describe strategies for implementing an evidence-based approach to clinical practice. Incorporate literature into your discussion. You may find some of the readings on the unit vUWS website useful to get you started. However, it is expected that you will conduct your own search for additional reading material. Marking criteria for the assignment are listed on the later part of this Learning Guide. 

PART B: Identification of evidence

Step 1: Clinical question and literature search
 
1. Write a clinical research question then complete the PICO table (Table 1) with a type of patients or problem, intervention, comparison intervention and outcome. The clinical research question should be extended from the discussion in part A 

2. Identify keywords and search terms to describe the problem, intervention, comparison intervention and outcome and write them in the table to assist you with your literature search.

Step 2: Conducting Search

1. Using the keywords and search terms identified in the table from Step 1 conduct an electronic database search.
