Jane is a 36-year-old female that has presented to the clinic where you work today. The GP has made a referral for the nurse to help write a chronic disease management care plan. Jane has recently been diagnosed with Type 2 diabetes and tells you that she is sick of the word diabetes and just wants to eat like she used to. As you are working through the care plan with Jane she talks about her work life, she divulges that she does not have a good relationship with her boss and works long hours to keep up with the job. You ask Jane how that makes her feel, and she states that she is having difficulty sleeping, concentrating, and is worrying all the time. Jane also states that she feels tense and feels down. Jane is struggling to manage her diet with her new diagnosis and is not taking her Diabetic medication regularly.  

You have conducted the following mental state assessment:
