Case Study Jane is a nursing student in year one of her Bachelor of Nursing Degree. She is undertaking her workplace experience (WPE) placement in a public hospital in rural New South Wales (NSW). Jane has a preceptor for her WPE placement, Steven. He is a clinical nurse specialist on the Medical Ward where Jane is undertaking her placement. It is Monday in the second week of a 10-day WPE placement for Jane. She has settled well into the Medical Ward and has received positive feedback from Steven on her formative assessment.

Jane is allocated to work on the day shift with Steven and they are caring for four (4) men in a four-bed room. Jane takes the vital observations for all four (4) men at 8am. She records them on the Electronic Medical Record (eMR) on the Standard Adult General Observation (SAGO) form on the computer. Jane notices that James Pearson in bed 4.3 has vital observations are outside of the flags and a notification comes up that they are in the yellow zone. Jane goes immediately to Steven who is talking to another man in bed 4.1 and asks if she can talk to him. She tells Steven away from the persons they are caring for that James Pearson has vital observations in the yellow zone. Steven, without checking any of the observations results, states “we do not do yellow zones here, don’t worry, just give him a glass of water, get him to take some deep breaths and take his observations in 30mins”. Steven deletes the vital observations that Jane has recorded in eMR.
