Jean Jean is a 68-year-old grandmother who was admitted to the Emergency Department (ED), University Hospital with two days of fever, a productive cough, myalgia and generalised malaise. She has a past medical history of type 2 Diabetes Mellitus, which she manages well with Metformin 500mg twice daily, Hypercholesterolaemia treated with Atorvastatin 10mg daily and Asthma treated with Salbutamol and Fluticasone inhalers. She is normally independent, active and lives with her husband. She is a non-smoker now, but smoked from 18-years to 43-years-old. You are assigned to look after Jean on her arrival to the Respiratory Ward from ED. She has been admitted with a diagnosis of a chest infection, and awaiting a CXR to confirm this. The medical team in the mean time prescribed IV antibiotics. Towards the end of your shift you notice she appears short of breath and using her accessory muscles. On assessment her SpO2 is 92% on room air, respiration rate is 27 breaths per minute, expiratory wheeze in all lungs fields and a weak cough. Blood pressure 110/60 mmHg, pulse 112 beats per minute and temperature of 37.5 degrees celcius
