Step 1: Read the following case study
Case study
John is a 20-year-old Aboriginal Torres Strait Islander man living with end-stage renal disease (ESRD). He normally lives in a remote Indigenous community and he is currently in a hospice 500 km from his home. John is receiving dialysis in the hospice, which the healthcare team have advised he requires to maintain his current health status. John has been told by the medical team he is not expected to survive more than three months on dialysis. John has decided that he wants to return home with no further treatment. The doctor has advised him that his life expectancy will be much shorter if he stops treatment.
You are the nurse who is looking after John. You are required to write a reflective essay that includes the following aspects:
Facilitation of collaborative ethical decision-making that incorporates the interdisciplinary team.
Cultural aspects of palliative care and in particular the practices, beliefs and traditions that John may wish to uphold.
Reflection of how your personal beliefs may impact on your professional interactions and the strategies you may use to manage this.
Step 2: Identify the ethical issuesLook over the information in Module 2 to get you started on your ethical issues readings. Access other resources and have a few readings to support your ideas on ethical issues. How might the multidisciplinary team support the ethical decision-making?
Module 3 talks about multidisciplinary care. Is there a particular team member that would be useful in this case study for John? You do not need to talk individually about each team member.
Step 3: Identify cultural considerationsWhat are some cultural considerations that John may have around death and dying? See the case study on Tom in Module 4. What issues are discussed? This is an area to start you thinking. Keep reading and find a few references that discuss cultural issues relating to John.
Step 4: ReflectReflect on how you may have different beliefs to John. How does this potentially affect your nursing care? How are you able to manage this to deliver the best nursing care for John? You can use first-person for this section if you wish. You still need to reference this—not your ideas but the measures you would use to overcome the differences.
