I have identified that wound measurement is not accurately documented at CTG clinic, Ontario, the place where I work. If a patient has more than one wound, nurses usually document “as multiple wounds” on the wound care flow sheet, instead of documenting wound size of each wounds on separate wound care flow sheets and care plan. The documentation of each wound size is important as it helps to identify whether the wound is healing within the four to six weeks’ timeframe, and the treatment is effective or not. In addition, continuity of the care can be achieved by appropriate documentation, which is a proof for any legal proceedings. To implement standardised practice of documentation of wound measurement for nurses at CTG clinic. I would provide survey questions to group of nurses who are working in the same facility. Use current policies and practices as resources, get management approval, review literature on wound measurement and frequency, education session to group of nurses. Chart audit to identify pre & post documentation practices.
I would ask the group of nurses’ responses, assess the data from the survey and the wound care flow sheet of the patients pre- and post-survey. I would also assess whether I have accomplished the goal by asking feedback from the manager if it is possible.
