NURSE: Good morning, my name is Katharine, I am the Practice Nurse here this morning. How can I  help you this morning? 
PATIENT: Well firstly I am sorry to get an emergency appointment like this, I do appreciate your  time, thanks for seeing me. I had bowel cancer just recently, I had an operation just over a week ago,  in fact it was slightly less than a week ago now. Everything has gone fantastically apparently, I am so  grateful for everything everybody’s done, they’ve been marvellous, and the people we met, possibly  having to have a stoma bag and things like this, but they’ve avoided that, I got away with it, very  very good. But the past few days I’m afraid…. I’m getting some pain in…. mainly in the big scar here.  There were a couple of others they did this side, but the big one here, it started to hurt and I think  possibly what’s making it hurt mainly is the fact that I……I’m constipated.  
NURSE: Right, OK. 
PATIENT: I don’t know quite why but…. and I’m straining a bit to just try and go but I daren’t strain  any more, I’m terrified I’m going to take the stitches out or destroy the work they’ve done. 
NURSE: Ok, Right. I’ve just got you notes here from when you’ve come out of hospital, so before we  progress any further can I just ask your full name and date of birth please? 
PATIENT: Its Keith Smith, 26th June 1957 
NURSE: So you’re clearly having some problems about opening your bowels? When did you actually  last have your bowels opened? 
PATIENT: A very tiny bit this morning. 
NURSE: Right. 
PATIENT: But it was a very tiny bit. 
NURSE: And can you describe what that looked like? 
PATIENT: Sort of….just a small….it was sort of solid, it wasn’t anything diarrhoea – ish or anything like  that, but small, solid lump if you know what I mean. 
NURSE: Would you say it was a bit like pebbles? 
PATIENT: Pebble, absolutely, yes. Sorry to whinge on as well… 
NURSE: No, its not a problem. 
PATIENT: The other problem is I’m so bloated across here. 
NURSE: I can see that you feel some discomfort. 
PATIENT: I feel so desperately uncomfortable, yeah and something is not right. 
NURSE: And its obviously causing you some problems so there’s absolutely no problem about you  getting checked out today.
