The avoidance of harm to research subjects should therefore be a prime consideration for all LSBE researchers and students. This ethics approval form must be completed by all UG and PG students who are undertaking a research project and be signed by both the student and the supervisor. If the research is not NHS-based, please complete the questions below and hand in the form to your supervisor (type written).
