CASE STUDY
Ms Nguyen was admitted into hospital after tripping over in a Melbourne city restaurant on Friday evening. Ms Nguyen was treated at the restaurant by bystanders until the paramedics arrived and was brought to the hospital emergency department.
Examination of Ms Nguyen identified: Sinus tachycardia of 110 bpm, Tachypnoea at 28 bpm, low oxygen saturations of 92% on room air with increased work of breathing and use of accessory muscles and exacerbation of existing lower back and limb pain secondary to a fall in the restaurant.
Ms Nguyen underwent a Chest x ray which identified extensive pneumonia. A lumbopelvic CT scan identified soft tissue injury and an echocardiogram was NAD. Blood tests identified low haemoglobin levels.
Ms Nguyen was then transferred to the medical ward for further management of her pneumonia and lower back injury and was commenced on intravenous therapy for rehydration.
The morning after Ms Nguyen’s admission, a reporter from a TV channel called the ward to find out what had happened to Ms Nguyen and how soon she would be discharged from the hospital. Ms Nguyen is a popular and well-known influencer and media identity who is regularly on TV.
Raphael, the registered nurse looking after Ms Nguyen that particular shift took the phone call. Raphael answered the reporter’s questions, revealing the tests and investigations that had been carried out and that Ms Nguyen might not actually be discharged from the hospital anytime soon because the doctors had not been able to identify a definitive diagnosis. Unknown to Ms Nguyen, this information was reported the reporter, on the morning TV show the following day.
Later that same day, two other nurses on the ward, Mia and Sam, were discussing how much they would love to have a selfie with Ms Nguyen for their Social media page. They were debating if they should go in and ask Ms Nguyen for her permission or wait until she is asleep and quietly take a picture without waking Ms Nguyen up. On entering Ms Nguyen’s room, they saw that she was fast asleep and decided not to wake her. Mia placed medications consisting of 10mg Endone, and 1gm Cephalexin in the medicine cup on the overbed table with a drink of water for Ms Nguyen to take when she woke. Having administered medications to her on the previous shift, Mia thought that she knew the patient well. Sam observed Ms Nguyen’s respirations and noted the work of breathing and rate had not changed much and thought they would still be the same as before and documented 22 bpm without counting them. The other vital signs have been recorded on the monitor and so she documented these. Sam and Mia quickly position themselves next to Ms Nguyen and quietly take a selfie photo before exiting the room.
Overnight, Ms Nguyen’s breathing became more difficult. Vital signs identified tachycardia of 130 bpm, Tachypnoea at 34 bpm, low oxygen saturations of 92% on 5L Oxygen and a MET call was initiated by the night shift nurse. Ms Nguyen was taken to ICU for treatment of consolidated pneumonia where her condition deteriorated. She did not respond to treatment, and died.
