This Agreement confirms the circumstances surrounding the termination of your employment with Long Cove Outdoors Inc., known as “Cove”. 

1. Termination Date
Your employment with COVE will terminate effective November 28, 2016 (Insert date notice was given to the employee).

2. Notice (choose the appropriate option and calculate according to legal requirements)
•COVE will provide you with a lump sum payment equal to three (3) week’s base salary, representing your notice and entitlement as prescribed under the Employment Standards Act, 1996. Payment will be made through the usual means.
OR
•The period between November 7, 2016 and November 28, 2016 is referred to in this agreement as the “notice period.” During the notice period your employment status will remain unchanged.  

3. Vacation 
Any outstanding vacation owing will be paid to you as a lump sum, less applicable statutory deductions. Payment will be paid through the usual means. 

4. Group Benefits
Your group benefits coverage, excluding the Paul Revere Long Term Disability, Blue Cross Travel Medical Insurance and Blue Cross Trip Cancellation/Interruption Insurance, will continue for three (3) weeks. (Insert an amount of weeks/months equal to the later of the ESA minimum or reasonable notice period).

5. Additional Payment [NOTE: Required if ESA minimums do not cover reasonable notice obligations or negotiated agreement with specific employee]

In addition to the above items and conditional upon the signing and returning of the attached Full and Final Release, COVE is pleased to provide you with an additional payment equal to (Insert number of additional weeks/months) three (3) months of base salary, paid through the usual means.

The offer contained in Section 5 is conditional upon your agreement to keep the terms hereof strictly confidential. You may only disclose the terms of this offer to your immediate family members and your legal and financial advisors. Any unauthorized disclosure of the terms of this offer shall immediately terminate the offer.
