Purpose:
The purpose of this assessment task is to apply specific assessment referral skills and relapse prevention planning for scenario clients who present with substance use, abuse and dependency issues.
Process:

Part A (350) word maximum for each client using paragraphs): Explore and explain the interactions between the addictive behaviours and the social context of the scenario client
Presenting Problems
Drug Use and Risk Assessment: E.g. any polydrug use and how smoking fits into lifestyle. Any suicidal ideation and ongoing risk management plan with other services if necessary. Any risk of harm to others during psychotic episodes etc.
Motivation to Change: What brought them to the program, what they are and aren’t willing to work on. What their long-term goals are
Predisposing Factors
E.g. lifestyle history of poor coping skills, high sensitivity to stress etc. Employment status, living conditions, social isolation etc.
Precipitating Factors
Why do they want help NOW? Any recent changes in their lives vocationally, interpersonally, physically or psychiatrically?
Perpetuating Factors
E.g. issues with impulsivity, cognitive insight and psychological mindedness, situational factors such as friends or family members who smoke, identified factors contributing to past relapses etc.
Protective Factors
Friends or family members who are supportive of them making changes, personal strengths, situational factors that may assist change process such as moving house, getting a new job etc.

Lucy is a 48 year old woman living in regional New South Wales. Lucy has recently started coming to counselling after losing her 19 year old son, Jacob, two months ago who died whilst holidaying with his friends in Bali. At the time of his death, Lucy had trouble sleeping and her brother, who is a GP, prescribed sleeping medication, which she has continued to take in ever-increasing doses the past two months. The cause of Jacob’s death is still unknown and the Balinese police have informed Lucy that the case is still ‘pending an investigation’. At the time of his death, Lucy and her estranged husband, Don, travelled to Bali and found Jacob in the hospital mortuary. Since returning home, a funeral has been held for Jacob and Don has returned to his home in Tasmania. Up until his death, Jacob, lived with Lucy and they had a close relationship. Lucy has no other children. During the time in Bali and immediate return to Australia, Lucy was busy making the arrangements and providing information to the Balinese police. At the time, Lucy feels as if she was numb throughout the experience, however, has recently lost weight, is struggling to concentrate at work as an emergency veterinarian and feels angry much of the time. Lucy has returned to a habit from her time as a university student and has begun taking small doses of anti-anxiety medication that is dispensed to dogs. Lucy feels guilty for helping Jacob pay for his trip as she did not think he was mature enough to travel at the time to Bali. Lucy feels as if her estranged husband, Don, has been able to “return to his life in Tasmania” with his new partner and child. Lucy is exhausted and constantly feels on edge and frustrated by the lack of information and miscommunication from the police in Bali. Jacob’s friends have tried to make contact with Lucy, but she is too upset when they call and text and does not answer or respond to their messages.
