Case

Abigail Chu is a registered nurse who works for Queensland Health in a school-based youth health service. In this service, registered nurses are placed in secondary schools to work with students, staff and parents. Nurses such as Abigail promote health and wellbeing, and connect students with health services, and make referrals to a general practitioner (GP), as required. Abigail’s job includes the provision of sexual health advice to young people who attend the service.

One day, a 16-year-old female student named Luna comes to see Abigail. During the consultation with Luna, Abigail learns that Luna is in a relationship with a boy named Tom from the same school and although they practise safe sex, Luna would like to start using the contraceptive pill to ensure she prevents pregnancy. Luna is already knowledgeable about different contraceptive options and, after discussions with Abigail for a short while, Abigail has noted that Luna appears to be quite mature compared to other students her age. Luna also has a good understanding of the benefits and side effects of the different contraceptive options. Luna would like to get a referral from Abigail to see the GP so she can get a prescription for the contraceptive pill.

Abigail has noted that Luna lives with her mother, her step-father Jim, and her younger brother. Abigail asks Luna if her parents support her relationship with her boyfriend and Luna tells Abigail that her family doesn’t know about her boyfriend at all. Abigail suggests that it might be a good idea to include her Mum in the decision-making about contraceptive options if possible. Luna appears upset at Abigail’s suggestion and says to Abigail, “Please don’t say anything to Mum. She might tell Jim and he gets angry.”

Abigail assures Luna that she won’t say anything to Luna’s Mum about her relationship if Luna doesn’t want her to know, but she encourages Luna to consider discussing the relationship with her Mum in the future. Following this, Abigail takes Luna’s blood pressure and notices some large bruises on Luna’s arm after she rolls up the sleeve of her shirt. Abigail is quite shocked and asks Luna how she got the bruises on her arm. At first, Luna just looks down and doesn’t say anything. Abigail then says, “Luna, you can tell me. Did your boyfriend do this?” Luna quickly responds saying, “No, Tom would never hurt me. Jim gets angry sometimes after he’s been drinking. You can’t tell Mum about my boyfriend. If she tells Jim, he’ll get angry again.”
