Her mother states “Mary went through a growth spurt over the last few months.” Mary plays soccer and loves to ride her bike and draw.  She does well in school, likes to receive praise for her marks, and likes to debate issues with friends and family. Mary’s teacher indicates that Mary has no difficulty solving complex problems and she pays attention in class. Lately Mary would rather be with her friends instead of doing her homework. Some of Mary’s friends have started puberty but Mary mother has not discussed puberty with Mary. Mary stays in her home without supervision after school as her parents’ work. Mary’s parents recently bought Mary a computer and she spends a lot of time surfing the net. Mary’s parents state “we want Mary to be independent but are worried about Mary as there is so much about peer pressure”.

Mr. Smith is an 80-year-old gentleman that lives with his daughter and her family.  They live in a 2-story single-family home with a flight of stairs between Mr. Smith’s bedroom and the main living area and bathroom.  The daughter had found Mr. Smith on the floor in the main living area when she returned home from work after not being able to reach Mr. Smith via telephone.  Mr. Smith was taken to the Emergency room for evaluation and other than some bumps and bruises was discharged later that day.  The daughter is concerned that Mr. Smith is unsteady on his feet and at risk for another fall.  She has accompanied Mr. Smith for his appointment at the community health center today and your role in the community health center is to provide outpatient education.
Vital signs are as follows- T- 37.0, P- 82, RR- 19, BP- 138/86, SpO2- 96%.
Mr. Smith has hearing aids bilaterally.
You have created a plan of care and a teaching approach for Mr. Smith in the previous assignment.  You have received a call at the Community Health Center from Mr. Smith’s daughter, and she is notably upset.  The plan that you have discussed with her, and her father is not being carried out in the community.  Home care has only come to see her father once in the last week and that was for the initial assessment and there has been no word from them after that.  The collaboration that you have set up with another healthcare professional has not been followed through.
