Assessment two allows you to demonstrate your knowledge of key case management concepts addressed within the subject, as well as your ability to apply them in practice.
For this task, you can choose to create a fictitious/anonymised case study based on your own practice or personal experience or the Maryn case study below. If you use your own case study please ensure that you do so from a case management perspective, that is, within the role of a case manager assisting a person attending the service. Additionally, if you are using your own case study please provide a short one to two paragraph overview of the agency's and person's (client's) situation on the cover sheet of your assessment. This summary is not included in the word count for the task.
Whether you use either your own (specified on the cover sheet) case study or the Maryn case study, your task is to:

• Assume that you are the case manager. What is your initial professional understanding of the person in your case study (or Maryn’s) presenting need/s at the intake stage.  Succinctly explain and justify your professional opinion.
• Describe your role as the case manager in assisting the person in your case study (or Maryn).
• Discuss how you would apply the overarching and phased functions of the case management model described in this subject and in the textbook, within the organisation and community context to your case study (or Maryn). This includes providing a reflective discussion on your approach to practice that focuses on decisions and reasons for the practice, the effectiveness of your practice and alternative approaches, skills or techniques (to demonstrate critical thinking).
• Demonstrate a person-directed case management approach to practice, with reference to the case study.
• Clearly outline and describe the communication skills you would use.

Maryn Case Study
Community Context Information: (The context and case study information is fictitious and has been developed for this assessment exercise. Please use this case study if you do not have a case study of your own.)
King Valley is a rural town of 15,000 population, surrounded by small mixed farming areas. Much of the local industry is related to agricultural supplies or supports and the general businesses required to support the everyday living. Local businesses include a chemist, grocery stores, medical surgery, hairdresser, clothing and shoe stores, several charity shops, church organisations, community health centre, and an Indigenous Health Service. One of the local church organisations offers a food bank service for those in need, financial counselling service free of charge and a free young mums support group. There are some private practitioners, a physiotherapist, an occupational therapist and two counsellors. All work part-time.The mental health services are provided by a team that is located in another town, Apollos, one hour away. Apollos is a regional centre of 55,000 people and it has a number of religious groups, major sporting organisations, charities and all governments services including DHS - Child Support and Centrelink, a child protection service, tenancy services and financial counselling services. There is a regular public bus service which runs weekly to Apollos, leaving King Valley at 8 am and returning at 6 pm on a Thursday. A Community Transport Service is sometimes available by appointment at a low cost to consumers. The King Valley local library has some public computer internet access available and offers skills development courses in computers and resume writing. Support groups like Alcoholics Anonymous regularly meet in the local library. There are two local high schools – a public and a Catholic high school. There are also two local public schools, again, a public and Catholic school. School counsellors for the Catholic schools travel from a nearby centre. There is a school counsellor attached to the public schools, but they are only available three days per week. The Public Schools have an Aboriginal Liaison officer and also offer quarterly parent information sessions on child development and a monthly behaviour management course.King Valley has some seasonal work available during summer, as some of the smaller agricultural farms produce fruit and vegetables and pickers and packers are regularly required. There is also an abattoir in tow, but it operates intermittently due to fluctuations in market demand. Rental properties are fairly costly at present because there is a major road reconstruction project occurring nearby.  Many of the workers for this project have taken medium-term rental leases locally as the project is expected to take two years to complete. There are limited Department of Housing properties available.There are several informal groups in town such as sporting clubs, ladies and craft groups, church-based support groups and a Rotary Club. The local facilities include several parks, a swimming pool and some good camping areas in the valley area just outside town.

There is an increase in tourism often in summer as it is a pretty valley known for good bushwalking and camping opportunities and is located around 2 ½ hours drive from the closest city.There is a council run child care centre for children five years and younger. The Centre offers after-school care. Both offer discounts for families receiving certain DHS- Centrelink payments.Much of the local population is of Anglo-Australian descent, there is a small Indigenous population in town.
With the contract road work, there has been an increase in the cultural mix in the area. Several of the construction workers have relocated their young families to the area with them for the duration of the project.

Organization Overview
You work full time as a case manager for children and families at the King Valley Family Centre, a non-government organization that applies a person-directed approach.
The King Valley Family Centre is funded by a range of State government programs and auspices a range of part-time services including drug and alcohol, family and adult counselors, a child and family service, and an interim service for people with disabilities. You are one of two case managers at the King Valley Family Centre and have good links with professionals in other organizations.

Referral and Information about Maryn
Maryn has self-referred as Maryn is feeling overwhelmed and does not know why.  Maryn is concerned about son Joe’s angry outbursts. Maryn drinks heavily and is motivated to reduce drinking alcohol.Maryn is a 33-year-old sole parent of three children aged 13 (Joe), 5 (James) and 2 (Shell) years, respectively.  Mick, the father of Joe, is an Aboriginal man and has been very involved in Joe's life since Maryn and the children returned to the area.  Mick works FIFO (Fly In Fly Out) and when home regularly has Joe for contact visits. Jim, the father of the younger two children, is in goal for fraud and is not due to be released for another four years. Maryn has ended the relationship with Jim about two years ago. Maryn has no contact with him. Maryn moved back to the local area approximately two years ago thinking that Maryn might get some help from Mick, which does happen when Mick is in town.Maryn rents their backroom room. This extra money helps pay bills. DHS-Centrelink is aware of the rental arrangement. Currently, a married couple (Barry and Betty) who are working on the roadworks are renting the room. They share the family bathroom, kitchen and lounge room. There is some concern about Barry's and Betty's drinking problems and arguments (unconfirmed). They owe Maryn four months rent and have not paid their share of the bills. Maryn has just received an electricity bill of $1000. When Maryn asked Barry and Betty for money for the electricity bill, they gave Maryn a bottle of wine and said they would pay soon. Maryn has been diagnosed with depression and is on a Disability Support Pension. Until recently Maryn would only drink when out with friends on Friday night. Maryn's drinking has increased in the last four months.  Maryn currently drinks five or six stubbies per night. Maryn often forgets to take the medication for depression.Joe has recently started truanting from school and Joe's school marks have suffered this year. Maryn is feeling unsupported in managing Joe’s truancy, as Mick is ‘always away working’. Mick feels that Mick's job is providing financial security for Maryn and Joe, through the child support that paid. Joe has a mild learning disability and receives some support at school through a teacher’s aide. Joe's behaviour is at times difficult as Joe lashes out when frustrated by not being able to do homework or not understanding something easily.Both Shell and James are becoming increasingly withdrawn. Both children suffer from asthma and food allergies. When Joe is at home, Joe sometimes helps with getting meals organised and housework. More recently Joe is 'out' with friends - wandering around town when Joe is not spending time with Mick. Joe does not like being at home because Barry and Betty are always arguing.Maryn has a few good supports in town. One of Maryn's friend's lives nearby. Maryn has started attending a free computer course at the library. Maryn is currently unemployed and would like to find some sort of work, part-time. Maryn used to work full time in an administrative job before meeting Jim. Maryn has had a series of six-month casual one day a week administrative jobs over the last few years, all of which have gone well. Maryn is known to drink less when "in work".
