On 5 May 98, WESTRALIA sailed from Fleet Base West for the Western Australia Exercise Area to rendezvous with HMA Ships SUCCESS, DARWIN and ADELAIDE. A fire in the main machinery space of WESTRALIA resulted in the loss of the lives of four crew and injuries to several other crew members. Prior to the ship sailing from Fleet Base West on 5 May 98, WESTRALIA had undergone an assisted maintenance period for about 6 weeks. During this period, members of the ship’s company of WESTRALIA in conjunction with Fleet Intermediate Maintenance Authority and the ship’s contractor, ADI Limited, carried out maintenance work. The work included the fitting of new flexible fuel hoses to the ship’s main engines by a subcontractor under the direction of ADI Limited.

After departing at 0900 on 5 May 98, at about 1030, when about 21⁄2 miles east of the Fairway Buoy in the Deepwater Channel, a fuel leak was noticed in the area of the number 9 cylinder on the inboard side of the port main engine. It was a significant leak, with fuel emerging under pressure in a manner similar to a garden hose.
