Task: Students are to identify a clinical incident involving medication management. clinical harm score 3-4. Access to incident reports should be negotiated with the student's workplace. If students are not currently employed alternatives will be discussed with the subject coordinator. Although Root Cause Analysis (RCA) is not required for clinical harm score 3-4 the RCA process will be used to review an incident systeniatically.
