Case study 1: Mia Masterson                                                                           

You are a registered nurse working in a local maternal and child health clinic. New parents David and Julian present with their 6 week old daughter Mia for a routine child health check. On assessment Mia appears to be developing well. Her height, weight and head circumference are within normal range. Mia is currently receiving formula via a bottle. During your consultation you notice that Mia is due for her first round of routine immunisations. You inform David and Julian of this. David states that Mia will not be receiving any immunisations. David adds that vaccines are merely used as toxin-based propaganda for large pharmaceutical companies to make money. Julian further informs you that his niece was diagnosed with autism not long after having one of her vaccines and that he and David will not take the same risk with Mia. 

Case study 2: Taylah Dennison                                                                         

You are a registered nurse working at the Junction Youth Health Service in Civic. One afternoon, Taylah (aged 15) presents with her best friend Cass. Taylah informs you that she attended a party at her friend’s house last month. Taylah adds that her friend’s parents were away. Taylah states she “got really pissed (drunk)” and that she engaged in consensual non-protected sexual intercourse with a male at the party. Taylah informs you that she is worried she might be pregnant as her last menstrual period was around six weeks ago. Taylah begs you not to tell her parents as her dad is in the army and will “loose his shit”.

With Taylah’s consent you provide her with a urine pregnancy test. The result is negative. As part of the consultation you obtain a sexual health history and suggest to Taylah that given her sexual activity, a screen for sexually transmitted infections would be appropriate. Taylah agrees and a vaginal swab is obtained. The following day the laboratory notifies you that Taylah is positive for chlamydia. You phone Taylah to inform her of the result and inform her that she will require antibiotic treatment and that she should contact all of her sexual partners as they too may have a chlamydia infection. Taylah is really worried about her parents finding out and asks you whether you need to tell them.

Case study 3: Jacob Scanes                                                                             

You are a registered nurse working in the ACT Health- School Nurse Program in an ACT government school. You are asked by the head of student services teacher to follow up with Jacob, a year 6 student who has reportedly been flat and disengaged in class. Jacob agrees to meet with you. During your discussion with Jacob, he discloses that his mother’s partner Dean has recently moved in with them. Jacob states it is now him, his mother (Amy), Dean, and Jacob’s younger brother (Jax) living in the two bedroom home. Jacob informs you that Dean comes home drunk from the pub a few times a week. Jacob adds that his mother and Dean have been arguing constantly about Dean’s alcohol use. Jacob informs you that last week when his mother and Dean were arguing, Dean pushed her against the wall and then slapped her against the face to “shut her up”. Jacob states that his mother sustained some bruising to the back of the head and some red marks to her left cheek but did not require medical attention. During your discussion you ask Jacob some questions around potential risks to safety. Jacob denies that Dean has ever been violent or abusive to either him or Jax. Jacob adds that he is “ usually a pretty cool guy”.
