You are expected to prepare a Change Management Plan based on the provided template and address all the required criteria. You have some flexibility in the format of your plan. As an alternative to Microsoft Word word report, you may create PowerPoint slides (maximum slides) with detailed notes under each slide or a self-recorded presentation video (a minimum of and a maximum of minutes).

Purpose

The purpose of this assessment task is to produce a basic and practical change plan which articulates how you will implement a specific change project in your/selected workplace. Your report should clearly synthesise the change you are intending to make and include a selection of relevant and appropriate tools. It also requires a roadmap with a timeframe for your change as well as other components.
