This assessment will be carried out during your acute clinical placement. You will apply Tanner's model of clinical judgement. The aim is to assess your ability to gather and integrate patient data to create a comprehensive patient care plan. Your essay must be supported by academic literature published within the last five years, and accurately referenced using APA guidelines.
 

TASK DETAILS/INSTRUCTIONS / STEPS
A. Noticing 

 Case Selection: Choose a patient case from your clinical practice. Discuss the significant details and observations that have implications for patient care and health outcomes.

B. Interpreting

i.Case Analysis: Analyse and interpret all subjective and objective information relevant to the patient, including the patient's health history, physical examination findings, and any relevant diagnostic tests.

ii. Differential Diagnoses: Develop five potential differential diagnoses based on your comprehensive case analysis. Each potential diagnosis should be justified with current, evidence-based scientific knowledge.

iii. Diagnosis and Pathophysiology: Using your evidence-based rationale, rule out four of the differential diagnoses to arrive at one probable diagnosis. Discuss the underlying pathophysiological processes at the cellular level of your chosen probable diagnosis.

C. Responding (Therapeutic Care Planning)

Identify three actual and one potential health problem related to your probable diagnosis. Following this, devise a comprehensive therapeutic plan of care for each identified problem. This plan should consider:

i.The patient's cultural, psychosocial, and spiritual influences.

ii.Specific, Measurable, Achievable, Relevant, and Time-bound (SMART) goals for each problem.

iii.Three evidence-based nursing interventions for each problem, supported by rationale from robust academic literature.

iv.The incorporation of pharmacological and non-pharmacological treatments. 

v.Strategies for patient education to enhance health literacy.

vi.Current literature and best practice evidence.

vii.Collaborative practice involving appropriate interprofessional referrals. 

viii.The formation of a partnership with the patient, whanau/family.

D. Reflection and Evaluation

Reflect on the entire patient care process, including your noticing, interpreting, and responding phases. Evaluate the effectiveness of your proposed interventions. Discuss what you have learned from this case and how it could influence your future clinical practice. Describe the markers of improvement to indicate success of the interventions.
