Monash Dental (MD) provides dental services such as fillings, scalings, extractions etc to its patients. These services are provided by a range of professionals (providers) that MD employ. Each provider is assigned a provider code. MD record the provider's name (as a simple attribute), the type of provider they are (for example general dentist, orthodontist, endodontist etc), the room number where they normally treat patients, the charges month to date that they have generated and the charges year to date that they have generated. Both these figures are an accumulation of the actual charges made by the provider month to date and year to date. For each service, MD record a service code, service description and the MD standard fee for this service. The actual fee charged to a patient for a particular service may be varied from this standard fee. Some services require a particular provider while other services are able to be provided by a number of providers. MD patients are assigned a patient number. The company records the patient's name, date of birth, the residential address (street, city, state, postcode) and contact phone number (all patients are required to provide a contact number for the purpose of confirming appointments). Some patients are covered by a medical insurer for dental treatment (a patient can only be covered by one insurer at any point in time). MD gives each such insurer an insurer code and record the company's name, address (street, city, state, postcode) and contact phone number. They also record the payments month to date and payments year to date which have been made by an insurer, on behalf of patients, to Monash Dental. Patients contact Monash Dental and make appointments to see a provider. Each appointment is booked with only a single provider, the provider is assigned when the appointment is first recorded. An appointment will require one or more services depending on the work required. For an appointment, MD record the appointment date and time and the room number in which the consultation (appointment) will take place - in some circumstances this room may not be the provider's normal room, for example, due to the need to use specialist equipment. The reception staff, based on the patient's requirements, set an appointment length as either short, standard or long. The staff who schedule appointments will also ensure that the allocated provider can provide the services which will be required during this appointment, your design is not required to enforce this, although these staff must be able to lookup which providers provide which services if required. MD has a strict policy that patients must pay for an appointment immediately on its completion. Non-insured patients will pay 100% of the cost of the appointment. Patients who have dental insurance will have part of their account paid by the insurer and will be required to pay the remainder (the balance), if any, of the account themselves. An account may be paid in full by the insurer depending on the policy the patient holds. Where a payment is made by an insurer, MD is only interested in recording that the payment was made by an insurer, they do not need to record which insurer made which payment. A unique receipt number is assigned to each payment. Providers require the assistance of a dental nurse during an appointment. Each nurse is assigned a nurse number - MD also record the nurse's name and their contact number. Some nurses have specialisations which they have been trained for such as anaesthesia or x-ray. These specialisations are used, where necessary, to determine which nurse is assigned to which appointment. At least one nurse is assigned to each appointment as the head nurse for that appointment. Depending on the work which is to be carried out, it may be necessary to assign a second nurse. MD does not assign more than two nurses to an appointment. The nurse assignments are scheduled by the clinics' admin staff usually during the day before the appointment.
