-Mr A is a 89 year old man RIBA Avg being found on the floor by daughter ,?unsure how long been on floor . Normally lives alone with his 2 elderly outdoor cats Daughter lives very close by and goes to his house morning and evening to help feed cats , check on holm , and bring him food . This is the house he has lived to since 1977 with his wife who passed away very suddenly in the home approx 10 years ago He has steadfastly refused any outside help ,declined conventional medical intervention and only just accepting some help from his son and daughter They have tried to get him to get hearing aids, fix his dentures and accept going into care but he has declined any offer of help . The family have noticed the deterioration over the past 8-9 months but particularly in the past 3 weeks . Daughter called ambulance 3 weeks ago but he refused to cope to hospital Ambulance personnel managed to convince him to agree on the admission . 
Relevant medical history None available as he was not engaged in medical service for the past 10 years. No regular medications or GP visits as he doesn't believe in them. 
Relevant investigation  CXR; indicated left lower lobe pnuemonia Influenza A8 and RSV PCR: negative/ Respiratory testing: NAD CRP 29mg/L (abnormal)/ V/BC and Neutrophils : NormaV CSU: Normal findings.
