Mr Cavalieri is a 75-year-old retired small business owner. Bruno is married to Rosa and they have five children, fifteen grandchildren, and two great grandchildren all of whom live in Melbourne. Bruno and Rosa live in their own home in Oakleigh, they receive no home help or community supports. Until recently, Bruno was still working two days a week at the family’s cafe, but due to health reasons, he has recently had to cease doing this. Mr Cavalieri’s oldest son Miko, has now taken over the family business.

Mr Cavalieri has a past medical history of hypertension (HTN), heart failure (HF) (New York Classification II, Ejection Fraction 45%), gastro oesophageal reflux disease (GORD), intermittent claudication, and carotid artery stenosis. Bruno is very adherent to his medical management plan and acts as a mentor to newly diagnosed patients with HF. He is currently on a 1.5L fluid restriction, has no allergies and is an ex-smoker (21 pack years). Mr Cavalieri drinks two to three glasses of Grappa each night after dinner. Over the last few days, Mr Cavalieri has been very disheartened over the change in his health status. Recently, he has been experiencing increasing episodes of chest pain and shortness of breath with minimal physical activity. At times, he has also been experiencing paroxysmal palpitations. The episodes of palpitations quickly settle once he stops what he is doing, sits down and catches his breath. Mr Cavalieri has also been complaining to Rosa of increased general malaise for the last 3 – 4 days. Unfortunately, his malaise has not been helped by a recent weight gain of 2kg and increased episodes of nocturia. On Thursday, Mr Cavalieri presented to the Swinburne Base Hospital Emergency Department with acute onset (< 1 hour) of pronounced weakness in his right arm, minimal weakness in his right leg, aphasia and homonymous hemianopsia. An urgent CT demonstrated an ischemic stroke involving his left middle cerebral artery. Mr Cavalieri was rapidly evaluated and deemed appropriate for treatment with Intravenous rT- PA. It is now 24 hours post treatment with rT-PA and Mr Cavalieri is progressing well on the 4th floor Stroke Unit of the Swinburne Base Hospital. He is admitted under Neurologist, Dr McGill.
Mr Cavalieri has a bag of N/Saline running at 8/24 rate via an 18G IVC in his left hand. Mr Cavalieri continues to have 2/24 urine output measures, 1/24 neurological observations and vital signs as per medical orders.
You are rostered on a morning shift and Mr Cavalieri is one of your allocated patients. You have three other patients allocated to you for this shift. They are all progressing well and require minimal nursing care.
