1. Mr Jones is a 93-year-old gentleman who sustained a fractured Left neck of femur  post mechanical fall in the garden. The patient is otherwise healthy and lives alone.  Mr Jones enjoys going for a nightly walk and looking after his cat. You receive Mr  Jones in the recovery after his surgery and note that the team have accidentally  operated on his right hip. 

2. Mrs Kumar has been on the ward for several hours post radical hysterectomy for  uterine cancer. Mrs Kumar has been reporting severe uncontrollable pain. The doctor  orders a CT scan and results show that medical sponges have accidentally been left  inside the patient. 

3. Ms Sally Winston has been brought to the ward after having extensive spinal surgery,  which lasted over seven hours. The patient was operated in the prone position. On  arrival to the ward you notice Sally has red marks on her cheeks and forehead and she  reports that her face feels like it is burning. 

The report needs to address the following questions:  

1. What is your patient safety issue and how does/could it occur? 

2. How common is your patient safety issue? (In Australia and or globally) o What are the impacts on patients when this type of incident occurs? 

3. What strategies are in place to reduce/prevent this patient safety incident occurring?
