Mr. Stan is a 59-years-old male who worked as an office administrator and he lives with his wife and one of his two children. He was a chronic smoker for the past 22 year years but stopped smoking 2 years before and having associated hypertension and diabetes rnellitus. His medical history also includes myocardial infarction two years ago and he is on medications. Five days before he got admitted to the hospital with chest pain and his coronary angiogram revealed a left coronary artery block with preserved left ventricular function. He underwent under-went Coronary Artery Bypass Grafting (CABG) and the graft was from the left saphenous vein. At present he is in cardiothoracic post-operative ward. He is complaining about incisional pain during cough with slight pain & swelling over the affected peripheral limb. He is positioned in half-lying with pillow under the left leg most of the time. He is reluctant and anxious to move his limbs. General examination reveals diminished thoracic mobility during breathing, shallow breathing pattern and reduced peripheral limb movements and he is anxious to take a deep inspiration Recently he was reporting about his decreased exercise tolerance and syncope while walking on level surtace.
