Case Study: Endometrial Cancer Mrs An Cheng, a 58-year-old woman, underwent a total abdominal hysterectomy (TAH) and bilateral salpingoopherectomy. She had recently developed post-menopausal vaginal bleeding. Following a pelvic examination and endometrial biopsy she was diagnosed with Stage IIIA Endometrial Carcinoma. She presented with no other health complaints. Social and spiritual history: Lives with her two sons (13 and 18 years old) and is recently widowed. Mrs Cheng works full-time as a Cantonese language teacher at a high school nearby. She has been teary about her diagnosis and future treatment. She is concerned for the future and the welfare of her children. She is a practising Buddhist, having immigrated from China 10 years ago. She has some extended family members living nearby, including her in-laws. It is now day 1 post-operatively, she has a vaginal pack and an indwelling catheter in-situ. The wound area on her abdomen is covered by a Comfeel® dressing and a combine, held in place with a supportive binder. She is worried about developing any complications post-surgery. Vital signs are stable, IV therapy and a Morphine PCA are in-situ. Intravenous antibiotic cover has been prescribed. You are required to discuss a holistic nursing care plan for her current nursing care and in preparation for discharge in 4 days' time according to the instructions below. The surgeon indicates she will need chemotherapy and radiation therapy in the near future. 

Case Study 2: Prostate Cancer Mr Giacomo Rossi, a 66-year-old man, underwent an open radical prostatectomy for Stage III prostate cancer following recent symptoms of urinary frequency, urinary tract infections and back pain. Previous medical history of hypertension, which is controlled with oral medication. Smoker for 30 years and drinks the occasional glass of red wine with meals. Social and spiritual history: Married to Maria for 40 years, immigrated from Italy 35 years ago. He runs his own family grocery business. A practising Roman Catholic, he attends church weekly. He has 4 grown up children with families who live nearby. Mr Rossi is reluctant to express how he is feeling about his diagnosis or treatment. It is now the morning of day 1 post-operatively. A 3-way-catheter with continuous irrigation is insitu. Urinary catheter output has been satisfactory, there has been moderate red urine output with some small clots. Urinary irrigation is due to be removed prior to discharge and the patient will go home with an indwelling urinary catheter and a leg-bag. Vital signs are stable. He has been complaining of some bladder spasms overnight and moderate pain. IV therapy and an Oxycodone PCA are insitu. Lower abdominal wound site is covered by an island dressing.
