You have arrived at the home of Mrs Bahia, a patient who requires non-emergency transport to three (3) concurrent medical appointments. 

Mrs. Bahia is a 70-year-old woman from India who has lived in Australia for the last 25 years. She experienced a stroke three (3) months ago and it was you, the non-emergency transport officer, who took returned her home following her discharge from hospital. 

The outcome of the stroke has left Mrs Bahia with right-side paralysis. It has limited her mobility in her right leg and allowed no use of her right arm. 

Her cognitive abilities remain intact and she recognises you from the previous patient transport encounter, approximately three months ago but her understanding of English is very limited. Mrs. Bahia has reverted to speaking in her native tongue most of the time. 

Mrs. Bahia is somewhat obese and has recently been diagnosed with type 2 diabetes. Her diabetes is the key purpose for her medical tests and appointments today. 

Mrs Bahia lives with her 50-year-old daughter, Wessal, who is documented as Mrs Bahia’s primary carer. 

Wessal is at home with Mrs Bahia when you arrive to transport her to-and-from the various appointments. 

On arrival you are invited into the home by Mrs. Bahia

On seeing Mrs Bahia, you are a bit taken back. Her health appearance seems to have deteriorated since her hospital discharge. She appears unkempt, with hair unruly and clothing grubby. You observe that she also appears anxious and upset, the house is very untidy with an odour of uncleanliness about it. 

Wessal, without moving from her recliner or taking her eyes off the TV in front of her, says hello to you and immediately launches in to say she is not escorting her mother today. She asks you to find out and let her know how she can get respite care for her mother. She then has no further communication with you as she continues watching TV. You are somewhat concerned with the actions and attitude of Wessal. 

Following the completion of the transport tasks for Mrs. Bahia, you return to ‘home base’ and join a new colleague and old friend for lunch. He asks you about your morning and because you are old friends outside the workplace and you are very frustrated, you tell him that you are concerned for Mrs. Bahia, believing her to be ‘at risk’ and that ‘obviously her daughter doesn’t care about her’, you provide more details of the interaction and patients circumstances than you should. 

Your friend and work colleague just laughs and tells you to ‘not to worry too much about it’ as it is a part of the Indian culture to take care of that sort of stuff by themselves and that ‘life’s too short to get involved with the clients’. 

You are left feeling uncomfortable with how the conversation went with your friend and colleague and know you have overshared and spoken out of order.
