Mrs Betty Green is a 69-year-old female who is transferred to the ICU2 (High Dependency Unit) from the general medical ward. She is two days post falling down three stairs where she sustained four right-sided rib fractures and a right-sided fractured neck of femur. She also sustained a large skin tear on her right arm.
She has been deteriorating for the past three hours and her observations on arrival to you are as follows: Spo2 92% on 3L oxygen via Nasal Prongs Pulse 105 beats/min Blood Pressure 105/55 Respiratory Rate 32 breaths/min Tympanic Temperature 37.8oC. Mrs Green has an indwelling urinary catheter insitu for fluid status monitoring. Her fluid balance chart shows a decreased urine output over the past 6 hours. Respiratory assessment reveals decreased air entry bilaterally. She has 2 peripheral Intravenous cannulas (IVC) in her right and left cubital fossa, one of the IVC’s is capped and not currently in use. You also note she has a dressing on her right arm and you cannot see in the notes when this wound was last reviewed. Mrs Green has a history of hypertension and has not yet had any of her daily medications including the mane Atenolol.
You are required to prioritise your nursing assessments and interventions in light of the patients’ current state and in association with any potential complications she may be at risk of developing.
