Mrs. Lillian Phillips is 84 years old and lives alone in her own home. Last night she was admitted to the local hospital following a fall in the bathroom. X-ray demonstrated she had fractured 2 ribs. She has a number of chronic illnesses (hypertension, macular degeneration, arthritis) which are managed by her General Practitioner and the Practice Nurse.
During the nursing health history Mrs Phillips' said she had been unwell for a couple of days, feeling dizzy and feverish and needing to pass urine frequently. Her daughter told the nurses that she was concerned her mother had not been eating well or drinking enough fluid and was spending a lot of time in bed. Physical assessment revealed severe bruising on her right shoulder and hip, she demonstrated non-verbal signs of pain (grimacing and moaning when moved) and said that the pain was 6/10 on movement and 3/10 when resting. There was evidence of excoriation of the genital area.
